Subjective
Client reports improved sleep over the past week, averaging 6-7 hours per night compared to 4-5 previously. States 'I actually feel like getting out of bed most mornings now.' Continues to endorse low motivation for social activities. Reports compliance with medication regimen (sertraline 100mg). Denies SI/HI.
Objective
Client arrived on time, dressed appropriately with improved grooming compared to initial sessions. Affect was brighter, with occasional smiles. Speech normal rate and rhythm. Engaged actively in session. PHQ-9 completed today: score 12 (moderate), decreased from 16 at last assessment.
Assessment
Client demonstrates continued progress in depressive symptom management. Sleep improvement is significant and appears to correlate with behavioral activation homework compliance. Social withdrawal remains the primary treatment target. PHQ-9 trajectory is encouraging. No safety concerns at this time.
Plan
Continue weekly individual therapy. Introduce graded social exposure tasks starting with low-stakes interactions. Review behavioral activation log next session. Maintain current medication. Coordinate with prescriber if plateau in 4 weeks. Next session: 03/06/2026.
