Continues to endorse low motivation for social activities. Reports compliance with medication regimen (sertraline 100mg). Denies SI/HI.

Objective

Client arrived on time, dressed appropriately with improved grooming compared to initial sessions. Affect was brighter, with occasional smiles. Speech normal rate and rhythm. Engaged actively in session. PHQ-9 completed today: score 12 (moderate), decreased from 16 at last assessment.

Assessment

Client demonstrates continued progress in depressive symptom management. Sleep improvement is significant and appears to correlate with behavioral activation homework compliance. Social withdrawal remains the primary treatment target. PHQ-9 trajectory is encouraging. No safety concerns at this time.
