Individual Therapy

Client reports ongoing difficulty adjusting to recent breakup and job change. States 'Some days I feel okay, and then something reminds me and I just crash.' Reports crying episodes 2-3 times per week, down from daily at intake.

Client arrived on time. Affect tearful at times but with broader range than previous sessions. Able to identify positive moments from her week when prompted. Speech normal. No suicidal ideation.

Adjustment disorder symptoms showing gradual improvement. The decrease in crying episodes from daily to 2-3 times weekly represents meaningful progress. Prognosis good.

Continue weekly therapy. Focus on building new social support network. Introduce values clarification exercise. Next session: 03/03/2026.
