Client reports stable mood over the past two weeks. States 'I feel pretty level, which is good for me.' Reports medication compliance with lithium and lamotrigine. No manic or hypomanic symptoms. Some residual low energy in afternoons.
Objective
Client presented in euthymic state. Affect congruent, range appropriate. Grooming and hygiene appropriate. Speech normal. Thought process logical and goal-directed. No psychomotor changes. Denies SI/HI.
Assessment
Bipolar disorder in stable phase with effective medication management. Residual afternoon fatigue may be medication side effect. Client engagement with mood tracking is a significant protective factor.
Plan
Continue biweekly individual therapy. Review IPSRT principles for maintaining circadian rhythm stability. Discuss afternoon fatigue with prescriber. Continue mood charting. Next session: 03/12/2026.
